Minimally Invasive Non-Surgical Therapy (MINST)
Minimally Invasive Non-Surgical Therapy, commonly shortened to MINST, is an advanced approach used to treat periodontal disease while causing as little disruption as possible to the surrounding gums and supporting tissues.
The treatment focuses on carefully removing plaque, calculus and harmful bacteria from beneath the gumline using small, precise instruments. Unlike periodontal surgery, MINST does not involve cutting the gums or placing stitches.

How is MINST different from conventional periodontal cleaning?
Conventional root surface debridement aims to thoroughly clean the affected root surfaces beneath the gums. MINST follows the same core principle but uses a more targeted and conservative approach.
Small hand instruments, fine ultrasonic tips and magnification may be used to access the affected areas while limiting unnecessary trauma to the gums. The aim is to preserve the blood supply, minimise tissue disturbance and create better conditions for healing.
What happens during treatment?
Your periodontal measurements and previous assessment findings will be reviewed before treatment begins. Local anaesthetic may be used to keep the area comfortable.
The affected root surfaces are then carefully cleaned using fine instruments designed to reach beneath the gumline. Treatment is concentrated on the diseased areas rather than aggressively instrumenting healthy tissue.
Depending on the number and location of the affected sites, MINST may be completed during one appointment or divided across several visits.
Will MINST hurt?
Local anaesthetic can be used to minimise discomfort during treatment. You may feel pressure, vibration or movement, but you should not experience sharp pain.
Because the approach is designed to reduce tissue trauma, some patients may experience less discomfort and gum recession than with more extensive treatment. However, tenderness and sensitivity can still occur while the gums heal.
What can I expect afterwards?
- Mild tenderness around the treated areas
- Temporary sensitivity to hot or cold
- Light bleeding during the early healing period
- Reduced swelling as inflammation settles
- Some gum shrinkage or teeth appearing slightly longer
The amount of discomfort and visible change varies between patients. You will receive personalised advice on cleaning and caring for the treated areas.
When will the results be assessed?
The gums need time to heal before the response to treatment can be measured accurately. A periodontal review will normally be arranged after an appropriate healing period.
At this appointment, pocket depths, bleeding and plaque levels may be recorded again. These findings will be compared with your previous measurements to determine whether the periodontal condition is becoming stable.
Can MINST help avoid periodontal surgery?
In some carefully selected cases, MINST may produce significant improvements and reduce the need for surgical treatment. It is particularly being investigated for deeper periodontal and intrabony defects.
However, it cannot guarantee that surgery will never be required. Some areas may remain too deep, inaccessible or unstable and may need further treatment or specialist assessment.
How can I support healing at home?
- Brush thoroughly twice each day using fluoride toothpaste
- Use the recommended interdental brushes or floss every day
- Follow any site-specific cleaning instructions provided
- Avoid smoking, as it can interfere with healing
- Attend all recommended review and maintenance appointments
- Contact the practice if you develop severe pain, swelling or persistent bleeding
